Zusammenfassung
Der arterielle Hypertonus ist ein bedeutender Risikofaktor für das kardiovaskuläre System und die Langzeitfunktion nach Nierentransplantation. Trotz potenter antihypertensiver Medikation persistiert bzw. aggraviert sich bei einigen nierentransplantierten Patienten die Hypertonie.
1994 führten wir die erste laparoskopische bilaterale Nephrektomie bei einer Patientin mit medikamentös nicht einstellbarem Hypertonus durch. Seither sind insgesamt 54 nierentransplantierte Patienten laparoskopisch bilateral nephrektomiert worden. Bei allen Patienten war vor Indikationsstellung eine subtile Differenzialdiagnostik erfolgt. Trotz Einnahme von 3–7 verschiedenen Antihypertensiva war der Blutdruck unbefriedigend eingestellt. Zur Einstufung des Schweregrades der Hypertonie wurde ein Medikamentenscore entwickelt, der die antihypertensive Potenz verschiedener Substanzen und Dosierungen berücksichtigt. Bei 48 von 54 Patienten kam es zu einer Normalisierung bzw. deutlichen Verbesserung der Hypertonie (mittlerer arterieller Druck) und bei 45 von 54 Patienten zu einer Reduzierung der Antihypertensiva (Medikamentenscore). Diese Veränderungen waren über alle Patienten jeweils statistisch hoch signifikant.
Die laparoskopische bilaterale Nephrektomie stellt bei nicht einstellbarer Hypertonie einen optimalen therapeutischen Ansatz dar. Insbesondere der postoperative Verlauf gestaltet sich extrem günstig, da sie im Gegensatz zu konventionell offen laparoskopisch operierten Patienten postoperativ keine Postaggressionsstoffwechsellage entwickelt. Obwohl individuell der Einfluss einer bilateralen Nephrektomie auf den Blutdruck prospektiv schwer abschätzbar ist, ist die sehr hohe Erfolgsrate (86%) im Hinblick auf das Langzeitorganüberleben sehr vielversprechend.
