Zusammenfassung
Zur Therapie der weitaus häufigsten Neuralgieform, der Trigeminusneuralgie, wurde eine große Zahl chirurgischer Methoden entwickelt und angewendet. Da die Ätiologie der Neuralgie seit der Erstbeschreibung 1756 [1] bis in die 6oer Jahre [8] unbekannt blieb, sahen die Eingriffe immer eine Unterbrechung der Schmerzleitung am Nerv vor, waren also ein ausnahmslos neurodestruktives Vorgehen. Von zentral nach peripher sind folgende Eingriffe durchgeführt worden oder werden z.T. noch angewendet:

die Ausschaltung im Kerngebiet des Hirnstamms, überwiegend für den 1. Ast nach Sjöquist [12]

2.
die parapontine Durchtrennung von 2/3 der sensiblen Trigeminuswurzel im Kleinhirnbrückenwinkel nach Dandy [3]
-
3.
die Kompression bzw. Dekompression des Nervs im Bereich des Cavum Meckeli nach Taarnhoj [15]
-
4.
die Durchtrennung einzelner Äste im Bereich des Ganglion Gasseri nach Spiller u. Frazier [13]
-
5.
die Thermokoagulation des Ganglion Gasseri nach Sweet u. Wepsic [14]
-
6.
die Alkoholinjektionen im Bereich des Ganglions nach Härtel [7].
