Summary
The midbrain is a highly vulnerable part of the brain and can be reached in standard microneurosurgery only with difficulties. Neuroendoscopy offers easier approaches to this area. Four case illustrations shall give some impression of this successful method. The first three patients underwent endoscopic interventions that combine third ventriculostomy and operative procedure in the region of the aqueduct. The last case of a 71-year-old woman was an endoscopic-assisted micro-surgical operation of an arachnoidal cyst of the cisterna quadrigemina. As a conclusion the endoscope is a useful tool for operations in the mesencephalic region without or with only a minimum damage to cerebral structures.
