Zusammenfassung
Der Pathomechanismus des chronischen Schulterschmerzes ausgehend vom subakromialen Raum wurde von Neer (1972) schon frühzeitig beschrieben: Die Einklemmung der Supraspinatussehne erfolgt im engen Raum unterhalb des coracoakromialen Bogens. Die Einengung kann vom Schulterdach, der Bursa, der Supraspinatussehne selbst oder dem Humeruskopf ausgehen. Die Faktoren können intrinsischer Natur (pathologische Veränderungen in der Sehne selbst) oder extrinsischer (Entstehungsursachen von außerhalb an die Sehne herangeführt) sein. Das daraus resultierende Impingement wird primär oder sekundär bezeichnet. Knöcherne anlagebedingte Formabweichungen des Akromions wurden von Bigliani beschrieben und in 3 Morphologietypen eingeteilt: flach (I), gekrümmt (II) und hakenförmig (III) (Abb. 1). Im Subakromialraum selbst kommt es durch den chronischen Entzündungsreiz zur Volumenzunahme der Bursa und Sehne mit Verklebung und Fibrosierung. Einlagerung von Kalkdepots in der Sehne des M. supraspinatus können zudem zu mechanischer Irritation beitragen. Vom Humerus ausgehend kann eine nach oben dislozierte Tuberculum majus Fraktur oder das Hochtreten des Kopfes selbst durch eine Schulterinstabilität oder einen Sehneneinriß (Codman 1934) bedingt, Ursache der Einengung des subakromialen Raums im Sinne eines sekundären Impingements sein. Neer klassifizierte das Impingement in 3 Stadien, reichend von Odem und Einblutung der Bursa und der Sehne der Rotatorenmanschette (Stadium 1).
