Zusammenfassung
Die Implantation der Totalendoprothese des Hüftgelenks stellt einen der erfolgreichsten Eingriffe der operativen Medizin überhaupt dar. Mit einer einfach durchzuführenden Operation gelingt es, dem Patienten ein schmerzfreies Gehen zu ermöglichen. Endoprothesen mit unbegrenzter Haltbarkeit stehen jedoch nicht zur Verfügung. Es besteht kein Zweifel daran, daß Endoprothesen möglichst klein sein sollten, bei der Implantation soll möglichst wenig Knochen reseziert werden. Kurze Prothesenschäfte, die eine inter-oder subtrochantäre Verankerung ermöglichen, schützen das Femur vor einer distalen Krafteinleitung und dem folgenden Stress-Shielding. Die Ergebnisse von modernen Implantaten, die eine geringe Knochenresektion erfordern, wie Schalen-oder Druckscheibenprothesen, müssen derzeit noch abgewartet werden. Derzeit gilt die Verwendung von Implantaten mit intramedullärem Stiel als zuverlässigste Verankerungstechnik. Entschließt man sich bei jungen Patienten zu einer Endoprothese, wird man in vielen Fällen ein spektakuläres Ergebnis beobachten. Man muß aber bereits vor der Erstimplantation die Notwendigkeit weiterer Eingriffe durch die spätere Lockerung der Prothese bedenken. Auch beim Prothesenwechsel sollten möglichst kleine Implantate verwendet werden, um das Femur für zwangsläufige Folgeeingriffe zu erhalten. Dem Operateur steht für diese Eingriffe eine breite Palette von unterschiedlichsten Implantaten zur Verfügung.
