Zusammenfassung
Zwischen 1984 und 1993 wurden an der Orthopädischen Universitätsklinik Heidelberg 54 Patienten (31 Mädchen, 23 Jungen) mit Myelomeningozele wegen einer Skoliose operiert. Der Skoliosegrad konnte von 90,9° auf 41,1° (55%), der Bekkenschiefstand von 20,9° auf 8,1° (61%) und der Rumpfüberhang von 27,9° auf 11,2° (59,9%) verbessert werden. Korrekturverluste, die nach einem durchschnittlichen Nachuntersuchungszeitraum von 2 Jahren und 7 Monaten auftraten, gehen im wesentlichen zu Lasten einer zu kurzen Spondylodesestrecke. Der Vergleich von 5 unterschiedlichen Operationsverfahren zeigt die günstigsten Ergebnisse bei einer kombinierten ventralen und dorsalen Instrumentation. Eine relativ hohe Komplikationsrate läßt sich deutlich reduzieren durch präoperative Ventilkontrolle, transpedikuläre lumbosakrale Verschraubung, ventrale Instrumentation, zusätzliche dorsale Stabilisierung mit LUQUE-Cerclagen, sorgfältige Wunddrainage mit ggf. primärer plastischer Dekkung des Zelenbereiches und durch frühzeitige Indikation zur Spondylodese.
