Zusammenfassung
Eine 65jährige, normalgewichtige Patientin mit seit 25 Jahren be-kanntem Typ-2-Diabetes mellitus stellte sich wegen plötzlich auftre-tendem Schwindel, Herzstolpern und Atemnot in der Ambulanz vor. Es fand sich ein Blutdruck von 165/95 mmHg sowie ein arrhythmi-scher Puls mit f=52/min. Das EKG zeigte Salven supraventrikulärer Extrasystolen. An diabetischen Folgeerkrankungen fand sich eine diabetische Retinopathie Grad II, diabetische Nephropathie im Stadium der Makroalbuminurie und eine symmetrisch sensible PNP beider Unterschenkel mit ausgeprägten Schmerzen, fehlendem ASR rechts, eingeschränktem Vibrationsempfinden und dystrophischen Hautveränderungen. Zusätzlich berichtete die Patientin über ein postprandiales Vollegefühl, Wechsel von Obstipation und Diarrhoe. Die deshalb durchgefuhrte Röntgenkontrastmitteluntersuchung des Magens (Abb. 12.1) zeigte eine verzogerte Motilität und Retention von Nahrungsbestandteilen. Nach Ausschluß anderer organischer Ursachen fiir die Herzrhythmusstörung (einschließlich Koronaran-giographie und Langzeit-EKG) und vorliegen einer manifester Neu-ropathie (sensomotorisch und autonom) wurde die Diagnose einer kardialen autonomen Neuropathie gestellt.
