Zusammenfassung

Die Mechanismen der Initiierung von Vorhofflimmern sind, soweit bis heute bekannt, vielfältig und sehr wahrscheinlich interindividuell unterschiedlich. Als mögliche Trigger werden Alterationen des autonomen Tonus sowie eine gesteigerte ektope atriale Aktivität diskutiert. Inhomogene langsame intraatriale und interatriale Leitungsgeschwindigkeiten und kurze atriale Refraktärzeiten gelten als elektrophysiologisches Substrat der Initiierung und Aufrechterhaltung von Vorhofflimmern (Allessie 1990). Auf der Grundlage dieser pathophysiologischen Erkenntnisse wurden nun in den vergangenen Jahren verschiedene Stimulations-verfahren zur Prävention von Vorhofflimmern entwickelt. Patienten mit paroxysmalem oder persistierendem Vorhofflimmern stellen dabei die Zielgruppe dieser neuentwickelten Schrittmachertherapien dar; die Indikationsstellung geht somit über die Behandlung assoziierter bradykarder Rhythmusstörungen hinaus.
