Zusammenfassung
Spasmolytika werden zur Lösung krampfartiger Schmerzen im Bereich von Magen, Darm, Gallenwegen, Harnwegen und des weiblichen Genitale eingesetzt. Wichtigste Gruppe sind die Anticholiner-gika (Antimuskarinika, Parasympatholytika), die Kontraktionen cho-linerg innervierter glatter Muskeln über eine Blockade muskarini-scher Acetylcholinrezeptoren hemmen. Hauptvertreter dieser neuro-tropen Spasmolytika sind Atropin, Scopolaminderivate und synthetische Anticholinergika. Einschränkend ist jedoch festzustellen, daß die therapeutische Wirksamkeit oral und rektal verabreichter Spasmolytika nicht ausreichend belegt ist, da viele der pharmakologisch wirksamen Substanzen aufgrund geringer Resorption oder hoher präsystemischer Elimination keine wirksamen Plasmaspiegel errei-chen. Diese Einschränkung gilt nicht für die natürlichen Belladonna-alkaloide Atropin und Scopolamin, die jedoch nicht zu den häufig verordneten Spasmolytika gehören.
