Zusammenfassung
Nackensteife (Meningismus) zeigt eine Irritation der weichen Hirnhäute an und gehört in Verbindung mit Kopfschmerzen zu den kardinalen Symptomen zahlreicher lebensbedrohlicher neurologischer und neurochirurgischer Erkrankungen. Nackensteife wird durch eine reflektorische Innervation der Halsmuskulatur hervorgerufen, um einer schmerzhaften Dehnung von Meningen und Spinalnerven entgegenzuwirken. Bei der klinischen Untersuchung imponiert Nackensteife als deutlich fühlbarer muskulärer Widerstand, der bei passiver Beugung des Nackens, aber nicht bei seitlichen Drehbewegungen des Kopfes auftritt. Zusätzliche meningeale Symptome sind die Zeichen nach Brudzinski und Kernig sowie in schweren Fällen ein Opisthotonus.
