Summary
Thirty-three shoulders (30 patients) with atraumatic avascular necrosis, operated on with an Aequalis shoulder arthroplasty between 1991 and 1994, were reviewed with an average follow-up of 18 months (minimum 12 months). There were 23 women and 7 men; at the time of surgery the average age was 57 years (35–81 years). Objectively the results were good with an average 40 point increase in the absolute Constant score (preoperative average 27 points, postoperative 67 points). The best results were observed in types II-IV necrosis (21 cases) with an absolute Constant score of 71 points (±12). In type V necrosis (12 cases) the results were less favourable, with an absolute Constant score of 62 points (± 16). Total arthroplasty (8 cases) yielded slightly better results for pain improvement and for mobility than hemiarthoplasty (4 cases) in these shoulders with glenoid erosion. Two sub-groups were identified: (a) radiation necrosis (2 cases), where the results were poor with persistent pain and reduced mobility; (b) complete humeral head necrosis in elderly women (4 cases), where the prosthesis resulted in a pain free shoulder in all cases although the mobility remained limited especially where there was a cuff tear.
