Summary
Glenoid resurfacing is technically challenging and this may explain why many surgeons still prefer to perform hemiarthroplasty. In our experience, total shoulder replacement has proven to give better functional results for many pathological conditions of the glenohumeral joint. Shoulder surgeons should be capable of performing glenoid resurfacing while anticipating technical errors responsible for most of glenoid component failures. Accurate preoperative planing using both standards radiographs and CT, as well as glenoid exposure are two key points. All the technical steps should be controlled including: (a) meticulous reaming with correct vertical and horizontal reorientation of the glenoid surface; (b) management of glenoid deficiencies; (c) correct positioning of the glenoid component; (d) secure fixation of the glenoid component; (e) finally, soft tissue reconstruction and balancing which should avoid any eccentric loads on the glenoid rim, responsible for glenoid lucent lines and loosening and for assymetric polyethylene wear.
