Zusammenfassung
Daß eine Dysfunktion der Tibialis posterior (TP)-Sehne zu einer progressiven Plattfußdeformität mit Schmerzen und verminderter Belastbarkeit führen kann, ist in den letzten Jahren verbreitet erkannt worden [5, 8, 7, 9, 14, 15, 16, 21]. Eine chronische Tendinose mit lokaler schmerzhafter Entzündung verbunden mit einer progressiven Ausdünnung und Schwächung der Sehne bis zur partiellen bis schließlich totalen Ruptur der TP-Sehne sind die Ursachen einer schrittweisen Destabilisierung des Fußlängsgewölbes; die sich einstellende Deformierung des Fußes ist gekennzeichnet durch eine Valgisierung des Rückfußes sowie Abduktion und Inversion des Vorfußes, einhergehend mit der Abflachung des Fußlängsgewölbes [9]. Die Fußde-formierung ist zunächst korrigierbar, wird im weiteren Verlauf aber zunehmend rigide und kann zum schmerzhaften lateralen Impingement bis zur Streßfraktur der distalen Fibula und schmerzhaften Arthrose von unterem und oberem Sprunggelenk führen [18].
