Zusammenfassung
Patienten mit patellofemoralen Beschwerden weisen oft eine Chondromalazie der Patella oder eine Fehlinsertion des Streckapparates auf. Dieser Theorie zur Folge ist es sehr überraschend, dass Patienten mit unauffälligen patellofemoralen Knorpelverhältnissen Schmerzsymptome entwickeln und andere mit ausgedehnten Knorpelläsionen völlig schmerzfrei sind. Bekannterweise ist hyaliner Knorpel nicht innerviert. Daher können Knorpelläsionen nicht die einzige Ursache für eine entsprechende Schmerzsymptomatik sein. Darüber hinaus können Patienten mit einem normal inserierenden Streckapparat ebenfalls an Knieschmerzen leiden. Zusammenfassend läßt sich sagen, dass die vorgenannten Theorien nicht die genaue Ursache des patellofemoralen Schmerzes erklären und dass andere pathophysiologische Prozesse existieren müssen.
