Zusammenfassung
Die erste bekannte interkorporelle Fusion wurde 1933 von Burns [3] bei einer kongenitalen Spondylolisthese am lumbosakralen Öbergang durchgeführt. Das dabei verwendete „Implantat„ war ein autogener Tibiaspan. In der Folge wurden sowohl an der Halswirbelsäule als auch an der Brustund Lendenwirbelsäule standardisierte ventrale Zugänge zum Intervertebralraum entwickelt, und auch heute noch stellt der autologe Knochen den „golden Standard„ für eine erfolgreiche Fusion dar. Cloward [4], Smith [15] und Robinson [13] etablierten die ventrale Fusion an der HWS mit zugerichteten Knochendübeln unterschiedlicher Form. Bereits in den 60er Jahren wurde jedoch das Problem der Primärstabilität des Knochenimplanta-tes deutlich, weswegen zur Fusion ein meist bioder tricorticaler Beckenkammspan verwendet wurde. Im Verlauf der knöchernen Durchbauung wurden aber auch hier Sinterungen und Resorptionsvorgänge des einge-brachten Knochens beobachtet, die das primäre Operationsergebnis durch eine sekundär entstehende Kyphosierung wieder teilweise zunichte machten. Ein weiteres Problem war und ist die sogenannte „Donor-site-morbidity„ an der Entnahmestelle. Auch bei einem standardisierten, technisch einfach durchzuführenden Verfahren wie der Beckenkammspanentnahme besteht eine Komplikationsrate von ca. 20%, wobei neben Hämatomen und Wundheilungsstörungen auch schwerwiegende Probleme wie Verletzungen des Nervus cutaneus femoris lateralis mit Meralgien und iatrogene Osteomyelitiden durchaus keine Seltenheit darstellen [22].
