Zusammenfassung
Zerebrale Sinusvenenthrombosen machen weniger als 1% aller Schlaganfälle aus, wobei die Mehrzahl der Patienten jünger als 40 Jahre ist. Da genaue Informationen zur Inzidenz der zerebralen Venenthrombosen aus epidemiologischen Studien fehlen, orientieren sich die Angaben zur Häufigkeit an den Daten prospektiver klinischer Serien, wo in Universitätskliniken mit einer Häufigkeit von etwa 5–8 Fällen pro Jahr gerechnet werden kann [5, 13]. Aufgrund der Tatsache, dass in der präangiographischen Ära die Diagnose nur autoptisch gestellt werden konnte, galt die zerebrale Sinusvenenthrombose lange als obligatorisch schwer und letal verlaufende Erkrankung. Moderne diagnostische Verfahren zeigen jedoch, dass Sinusvenenthrombosen häufiger vorkommen als früher angenommen und eine deutlich bessere Prognose aufweisen. Aufgrund der Vielfalt der klinischen Symptome und der Variabilität des Verlaufs wird die Diagnose jedoch oft zu spät gestellt oder ganz übersehen. Wichtig ist daher, bei Schlaganfällen mit fluktuierendem Verlauf, Kopfschmerzen, epileptischen Anfällen und Bewusstseinsstörungen eine zerebrale Venenthrombose in die differentialdiagnostischen Überlegungen mit einzubeziehen. Bei frühzeitigem Therapiebeginn liegt die Letalität von blanden Formen heute deutlich unter 10%. Thrombosen der tiefen inneren Hirnvenen und septische Thrombosen haben demgegenüber auch weiterhin eine hohe Mortalität.
