Zusammenfassung
Frakturen des proximalen Humerus machen einen Anteil von 5% aller Frakturen aus. Die Mehrheit dieser Frakturen wird als minimal disloziert beschrieben und können konservativ mit gutem funktionellem Ergebnis behandelt werden. 15% der proximalen Humerusfrakturen werden allerdings als disloziert beschrieben. Ein großer Anteil dieser Frakturen benötigt einen operativen Eingriff entweder in der Form einer Osteosynthese oder einer Endoprothese, um ein gutes funktionelles Ergebnis zu erreichen. Es ist daher besonders wichtig, zwischen Patienten, die konservativ behandelt werden können und denen, die einen operativen Eingriff benötigen, unterscheiden zu können. Um Behandlung und Prognose auf die individuelle Fraktur abstimmen zu können, hat Neer 1970 eine 4-Fragment-Klassifikation entwickelt, die auf der Intaktheit folgender Fragmente beruht: Humeruskopf intraartikulär, Humerusschaft, Tuberculum minus und majus. Diese Klassifikation ist bis heute die am meisten benutzte Einteilung (Neer 1970). Damit das Fragment als verlagert gilt, muss es folgende Kriterien erfüllen: Verlagerung um mehr als 1 cm oder Rotation um mehr als 45 Grad. 2-, 3- und 4-Fragment-Frakturen und Luxationsfrakturen werden in dieser Klassifikation beschrieben, wobei die 2-Fragment-Fraktur am häufigsten auftritt. Frakturen am „collum anatomicum“ und „collum chirurgicum“, wie Frakturen der Tubercula minus und majus werden als 2-Fragment-Frakturen gewertet. Da dies eine radiologische Klassifikation ist, ist es absolut notwendig Röntgenbilder zu veranlassen, die zumindest aus einer antero-posterioren, lateralen und einer axillhumärusren Aufnahme bestehen müssen, um eine akkurate Diagnose stellen zu können (Neer 1984, Sidor et al. 1994).
