Zusammenfassung
Die zunehmende Bedeutung der endoprothetischen Versorgung und die Verbesserung, anderer rekonstruktiver Eingriffe schränkt die Indikation zur Schulterarthrodese immer weiter ein (Cofield und Briggs 1979, Wilde et al. 1987). Dennoch stellt dieses Verfahren auch heute noch eine wichtige Methode der Schulterrekonstruktion für spezifische Indikationsstellungen dar und ermöglicht es, dem Operateur, dem Patienten zu einer stabilen, belastbaren Schulter zu verhelfen. Das hier vorgestellte konventionelle Verfahren der Schulterarthrodese eignet sich für die meisten Indikationsstellungen. Stabilität und Belastbarkeit der Schulter werden jedoch mit dem Verlust der gesamten glenohumeralen Rotationsfähigkeit erkauft. Wo immer möglich sollte daher einer endoprothetischen Versorgung der Vorzug gegeben werden. Die meisten Autoren betrachten das Vorliegen eines Schlottergelenks als eine Indikation zur Schulterarthrodese. Auch bei Poliomyelitis, Abriss der proximalen Armplexuswurzeln, irreparablen oberen Armplexusläsionen und gelegentlich bei isolierter Lähmung des N. axillaris kann eine Schulterarthrodese in Frage kommen. Diese Patienten verfügen über eine ausreichende Ellenbogenund Handgelenksfunktion, können aufgrund der fehlenden Fähigkeit, die Handposition im Raum zu steuern, die Funktion der oberen Extremität jedoch nicht optimieren (Richards 1991, Rouholamin et al. 1991). Liegt in einem solchen Fall eine gute Funktionsfähigkeit der Schulterblattmuskulatur, insbesondere des M. trapezius, M. levator scapula und M. serratus anterior vor, führt die Arthrodese zur Stabilisierung der Extremität und ermöglicht den effektiven Gebrauch der Hand in Arbeitshöhe.
