Zusammenfassung

Codman berichtete erstmals 1911 über die operative Rekonstruktion der Rotatorenmanschette. Er hielt frühe Diagnosestellung und Rekonstruktion der Ruptur für die beste Möglichkeit die Funktionalität der Schulter wiederherzustellen und den Patienten frühzeitig wieder in das Arbeitsleben zu reintegrieren (Codman 1934). Die Selektion der Patienten und Indikationsstellung zur offenen Rekonstruktion ist allerdings bis zum heutigen Tag kontrovers. Konservative Therapie mit Krankengymnastik und Medikamenten erzielen inkonsistente Resultate. Zufriedenstellende Ergebnisse werden bei Patienten mit niedrigem funktionellen Anspruch in 44–82% erreicht (Samilson und Binder 1975, Cofield 1985, Itoi und Tabata 1992, Bokor et al. 1993, Gazielly et al. 1994).
