Zusammenfassung
Herz und Gefäße bilden ein in sich geschlossenes Transportsystem, in dem das Blut als Transportmittel zur Ver- und Entsorgung der Körperorgane dient und daher in einer bestimmten Menge mit einem den jeweiligen Bedürfnissen entsprechenden Druck befördert werden muss. Das hierzu notwendige Druckgefälle wird im Körperkreislauf durch die Kontraktion der linken Herzkammer erzeugt und durch das nachgeschaltete, elastische Gefäßsystem moduliert. Während der kardialen Erschlaffungsphase (Diastole) wird der Fluss des Blutes durch die sog. Windkesselfunktion der großen arteriellen Gefäße, insbesondere der Aorta, aufrechterhalten. Hierbei zieht sich die in der Systole durch das Herzschlagvolumen gedehnte Gefäßwand der Aorta in der Diastole aufgrund ihrer elastischen Eigenschaft wieder zusammen, so dass das Blut kontinuierlich entsprechend der Richtung des geringsten Widerstandes in die Peripherie weiterbefördert werden kann. Das periphere arterielle Gefäßbett ist durch eine nach distal zunehmende Ansprechbarkeit auf neurale und humorale Impulse befähigt, durch Kontraktion oder Dilatation der Gefäßmuskulatur den Lumendurchmesser zu variieren. Besonders im Bereich der zahlreichen Arteriolen bewirken Veränderungen des Gefäßmuskeltonus erhebliche Auswirkungen auf den Gesamtquerschnitt und somit auf den Strömungswiderstand, so dass die Aktivität der glatten Gefäßmuskulatur in diesen Abschnitten der entscheidende Faktor für die Regulation sowohl der nachgeschalteten kapillären Organdurchblutung als auch der vom Druck abhängigen Durchblutung im gesamten arteriellen Gefäßsystem ist.
