Zusammenfassung
Mit der zunehmenden alloarthroplastischen Versorgung auch jüngerer Coxarthrosepatienten sowie der Verwendung neuer Gleitpaarungen wie der Keramik-Keramik-Paarung gewinnt der Faktor „postoperatives freies Bewegungsausmaß“ zunehmend an Gewichtung. Anders als beim senilen Coxarthrotiker ist das nach der Endoprothesenimplantation zur Verfügung stehende freie Bewegungsausmaß für den jungen Patienten von gewichtigem Interesse. Mit der Keramik-Gleitpaarung ist der Kantenbelastung besondere Beachtung zu schenken, so dass hier eine exaktere Implantation notwendig ist. Auch bei den übrigen Hüftendoprothesen stellt die Dislokation neben der Infektion eine der postoperativen Hauptkomplikationen dar. Verzögerte Mobilisation, verlängerte Rehabilitationszeit oder, falls rezidivierend, sogar Revisionseingriffe sind die Folge (Daly/Morrey 1992). Dabei treten nicht unerhebliche Belastungen sowohl für den Patient als auch für den behandelnden Arzt auf (Bruce et al. 2000). Die Prävalenz der Gelenkinstabilität beträgt in neueren Studien 1,5–4% aller hüftendoprothetischen Erstversorgungen (Li et al. 1999; Yuan et al. 1999), wobei diese Zahl bei Revisionseingriffen auf bis zu 26% ansteigen kann (Fraser/Worblewski 1981; Kavanagh 1987; Kavanagh/Fitzgerald 1985) und somit erhebliche sozioökonomische und psychologische Konsequenzen haben kann. Die Patienten haben darüber hinaus eine behindernde Einschränkung der Lebensqualität und leben in einer steten Angst und in Erwartung der nächsten Luxation (Chandler et al. 1982; Gächter 1989).
