Zusammenfassung
Die chirurgische Behandlung der terminalen Herzinsuffizienz gewinnt zunehmend an Bedeutung. Als Ursachen hierfür sind zum einen quantitative demographische Veränderungen der Patientengruppe und zum anderen die keineswegs lediglich durch das Spenderaufkommen bedingte Stagnation im Bereich der Herztransplantation zu nennen. Die Herztransplantation einer kleinen, hoch selektierten Patientengruppe zeigt gute mittelfristige Ergebnisse mit einer heute allgemein erreichten 1-Jahressterblichkeit von 15-20% und einer etwa 50-60%igen Chance, 5-7 postoperative Jahre zu erleben [7]. Lebenslange Immunsuppression als Gratwanderung zwischen Infektion und Rejektion sowie die im Langzeitverlauf zunehmende Transplantatarteriosklerose bestimmen jedoch erheblich die postoperative Lebensqualität und -erwartung.
