Zusammenfassung
Seit den frühen Berichten über die Möglichkeiten der Lungentransplantation hat sich diese Therapieform weit entwickelt [4, 21]. Sie ist nun eine etablierte Behandlungsmethode für ausgewählte Patienten im terminalen Lungenversagen [3, 9]. Die 1-3-Jahresüberlebensraten liegen bei 78 [19] bis 83% [6]. Mit zunehmender Akzeptanz der Lungentransplantation wuchs aber auch der Bedarf an adäquaten Spenderorganen. Lediglich bei 10-20% der Multiorganspender kommen jedoch die Lungen für eine Transplantation in Frage [22]. Der Bedarf übertrifft bei weitem die Anzahl der zur Verfügung stehenden Organe, wodurch die Wartezeit auf ein Organ zunehmend länger wird [13]. Mittlerweile versterben bis zu 50% der Patienten auf der Warteliste [18]. Der Mangel an Spenderorganen ist der wesentliche Faktor welcher eine ausgedehntere Nutzung der Lungentransplantationen verhindert. Ein Grund für die niedrige Anzahl von Lungenspendern sind dabei die strengen, historisch gewachsenen Kriterien zur Akzeptanz von Lungenspendern [10].
