Zusammenfassung
Die negativen Auswirkungen der extrakorporalen Zirkulation auf den Gesamtorganismus sind bekannt und werden als generalisierte Entzündung im Sinne eines „systemic inflammatory response syndrome“ (SIRS) verstanden (1, 4, 6). Zusammengefasst liegt bei Komplementaktivierung eine komplexe immunologische Antwort, insbesondere auf den Kontakt mit großen Fremdkörperoberflächen, vor. Pathophysiologisch resultiert in erster Linie eine erhöhte Kapillarpermeabilität mit Ödemneigung, Blutgerinnungsstörungen sowie Infektanfälligkeit und Schädigung der Erythrozyten. Der postoperative Verlauf wird durch das Syndrom häufig stark negativ beeinflusst, insbesondere wenn pulmonale, renale oder neurologische Insuffizienzen zu therapieren sind (5).
