Summary

Primary spontaneous intracerebral hemorrhage is a frequent and sever illness with imminent risk of death and dependence. There seems to be a relatively poor scientific interest in this disease, compared with ischemic stroke. Only a small number of controlled, randomized trials were conducted in intracerebral hemorraghage and still there are many ocntroversies about treament. This concerns the basic treatment and the specific treatment of hematomas. The limited number of controlled, randomized studies in spontaneous intracerebral hemorrhage severly limit positive recommendations for any intervention, for blood pressure management or for intraventricular fibrinolysis.The actual knowledge about specific treatment options in spontaneous intracerebral hemorrahge is reviewed in this article and an attempt is made, to give recommendatiions for different clinical features.

Zusammenfassung

Die spontane intrazerebrale Blutung steht trotz ihrer Häufigkeit und schwerer klinischer Verläufe mit drohender Behinderung oder Pflegeabhängikeit im Schatten der ischamischen Hirninfarkte. Wah rend eine Vielzahl klinischer Studien die Behandlung des is chämischen Hirninfarkts zu optimieren versucht und ein erheblicher Erkenntniszuwachs bezüglich der Ätiopathogenese und Therapie in den letzten Jahren zu verzeichnen war, ist das wissenschaftliche Interesse an intrazerebralen Blutungen verhältnismäßig gering. So ist es nicht verwunderlich, dass seit vielen Jahren Kontroversen in der Behandlung spontaner intrazerebraler Blutungen bestehen, die sowohl vermeintliche basistherapeutische Maßnahmen wie die Blutdruckbehandlung betreffen als auch spezielle Therapieverfahren wie die operative Hämatomevakuation oder Behandlung intraventrikulärer Blutclots. Es soll ein Überblick über den aktuellen Stand der speziellen Behandlung sogenannter primärer spontaner intrazerebraler Blutungen gegeben werden und der Versuch gemacht werden, Entscheidungshilfen für den klinischen Alltag zu geben.
