Zusammenfassung
Die elektrische Induktion eines therapeutischen Krampfanfalles (EKT) war nicht das erste konvulsive Verfahren (Kap. 1). Zunächst hat man mit Analeptika wie Kampfer und später Cardiazol generalisierte epileptische Krämpfe ausgelöst. Sie waren in den frühen Jahren ohne Narkose und Relaxation sowie ohne adäquate Überwachung sehr gefährlich. Nachdem einzelne Patienten während der chemischen Konvulsionstherapie gestorben waren, wurde von Cerletti ab 1938 die wesentlich besser steuerbare elektrische Induktion mit 50 Hz Wechselstrom eingesetzt (zur Übersicht: Kalinowsky u. Hippius 1972). Die große Insulinkur mit Hypoglykämie bis zum Koma (insulin coma therapy) und Krampfanfall, 1934 begründet, wurde bis in die achtziger Jahre des 20. Jahrhunderts ausgeführt. Mehrere Zwischenfälle und die schlechte Steuerbarkeit drängten sie jedoch in den Hintergrund (Cramond 1987; Holden 1999). Aufgrund der moderaten Wirksamkeit der bisherigen subkonvulsiven rTMS bei melancholisch depressiven Patienten (Eschweiler et al. 2001; Loo et al. 1999), insbesondere bei psychotisch depressiven (Grunhaus et al. 2000) und älteren depressiven Patienten (Figiel et al. 1998), wird weiterhin nach schonenderen Applikationsformen der konvulsiven Therapie gesucht.
