Zusammenfassung
Karbon in der Behandlung der Arthrose ist seit 1983 im Gespräch, seit Minns und Brittberg ihre Arbeiten zum Einsatz von Pads und Stiften in den Knochen geschädigter Knorpelareale veröffentlichten [1–3]. Sowohl im Tier-Experiment als auch im klinischen Einsatz beim Knorpeldefekt des menschlichen Kniegelenkes zeigten die Arbeiten, dass Karbonfasern die Neubildung von Faserknorpel induzieren kann [2–5]. Das entstandene Gerüst aus Karbonfasern und Ersatz-Knorpel war mechanisch so stabil, dass die Knochenoberfläche auch unter Belastung dauerhaft bedeckt blieb. Seit 1993 hat Schweinsberg die Karbonfaserstifte vor allem bei 4-gradigen Knorpelschäden menschlicher Kniegelenke eingesetzt. Dabei hat er die geschädigten Knochenareale mit einem 3,2 mm Bohrer 16 mm tief aufgebohrt und 3 mm dicke und 12,5 mm lange Karbonfaserstifte implantiert. Pro cm2wurde ein Stift verwendet. Die Patienten durften sofort wieder belasten und nach 1 Jahr waren 51% der Patienten ohne Beschwerden, Langzeit-Ergebnisse sind nicht bekannt. Der Eingriff wird arthroskopisch und ambulant durchgeführt.
