Wertigkeit und Bedeutung der Narkosemobilisation

Zusammenfassung

Das Problem der Schultersteife wird erstmals von Duplay (1872) beschrieben, Codman (1934) charakterisiert das Schulterproblem als „schwierig zu diagnostizieren, schwierig zu behandeln und schwierig zu erklären“. Dies trifft auch noch heute zu. Man unterscheidet die primäre von der sekundären Schultersteife. Der Begriff der Periarthritis humeroscapularis, frozen shoulder, adhäsive Kapsulitis u. a. wird zwar heute durch andere diagnostische Begriffe ersetzt, allzu leicht aber durch den Begriff des Impingement-Syndroms, mit dem eine differenzierte Diagnose aber wieder verallgemeinert wird. Es ist nicht einfach die adhäsive Kapsulitis, feingeweblich gesehen eine entzündliche Veränderung der Schultergelenkkapsel einschließlich der umgebenden Bänder, von sekundären Schultersteifen zu unterscheiden. Während bei sekundären Schultersteifen in der Folge der Schultereckgelenksarthrose, beim degenerativen Rotatorendefekt u. a., aber auch bei der Tendinosis calcarea, nach erfolgloser konservativer Therapie in aller Regel die Arthroskopie mit nachfolgender operativer Versorgung resultiert, so ist es bei der primären Schultersteife von Bedeutung, den richtigen Zeitpunkt des invasiven Vorgehens zu wählen.
