Zusammenfassung
Trotz umfangreicher Bemühungen im Bereich der Prävention ist die Prävalenz von Frühgeburten in den meisten westlichen Ländern in den letzten 10 Jahren weitgehend konstant geblieben. Die Perinatalsterblichkeit hat dagegen deutlich abgenommen, nicht zuletzt als Folge einer erheblichen Verbesserung der Überlebenschancen von kleinen Frühgeborenen mit einem Geburtsgewicht von < 1500 g und < 1000 g (Hack et al. 1995; Strebel u. Bucher 1994). Nur etwa 1 % aller Lebendgeburten gehören zu der Gruppe der sehr kleinen Frühgeborenen. Dennoch kommt dieser Gruppe wegen dem mit der Betreuung verbundenen Aufwand sowie dem hohen Anteil an der perinatalen Mortalität und Morbidität eine besondere Bedeutung zu. Die optimale Versorgung dieser Hochrisikofälle ist nicht nur für die betroffenen Familien, sondern auch für die Gesellschaft als Ganzes von großer Tragweite und stellt die Verantwortlichen sowie auch die Eltern immer wieder vor schwierige medizinische, ethische und soziale Fragen.
