Zusammenfassung
Spasmolytika werden zur Lösung krampfartiger Schmerzen im Bereich von Magen, Darm, Gallenwegen, Harnwegen und des weiblichen Genitale eingesetzt. Wichtigste Gruppe sind die Anticholinergika (Antimuskarinika, Parasympatholytika), die Kontraktionen cholinerg innervierter glatter Muskeln über eine Blockade muskarinischer Acetylcholinrezeptoren hemmen. Hauptvertreter dieser neurotropen Spasmolytika sind Atropin, Scopolaminderivate und synthetische Anticholinergika. Während die natürlichen Belladonnaalkaloide Atropin und Scopolamin eine gute Bioverfügbarkeit aufweisen, ist die therapeutische Wirksamkeit vieler synthetischer Anticholinergika nur nach parenteraler Injektion, aber nicht nach oraler oder rektaler Gabe ausreichend belegt, da viele der pharmakologisch wirksamen Substanzen aufgrund geringer Resorption oder hoher präsystemischer Elimination keine wirksamen Plasmaspiegel erreichen.
