Zusammenfassung
Fall 1. Bei einem 45 jährigen Bankkaufmann wird anläßlich einer Einstellungsuntersuchung eine Blutdruckerhöhung auf 158/106 mmHg festgestellt,die sich bei späterer Kontrolle bestätigt. Der Patient gibt an,daß auch sein Vater seit dem 50. Lebensjahr an Hypertonie leidet. Er selbst hatte keine Beschwerden. Als Risikofaktoren bestanden: Nikotinabusus, mäßiger Alkoholkonsum (3–5 Flaschen Bier pro Abend) und ein 20%iges Übergewicht. Der übrige physikalische Befund ist unauffällig. Bei der Basisdiagnostik wird eine Lipidstoffwechsel-störung (Cholesterin 260 mg%,Triglyzeride 380 mg%) und ein erhöhter Nüchtern-Blut-zucker (135 mg%) gemessen. Diagnose: Arterielle Hypertonie vermutlich essentieller Genese, metabolisches Syndrom — die häufigste Form der Hypertonie, die zunächst durch Allgemeinmaßnahmen wie Gewichtsnormalisierung, Reduktion des Alkohols, körperliche Aktivität, salzarme und cholesterinarme Ernährung therapiert werden sollte.
