Zusammenfassung

Fall 1. Der 27 jährige, nicht seßhafte Patient wurde mit akuter Dyspnoe und Fieber unter dem Verdacht auf eine Pneumonie eingewiesen. Multiple Einstichstellen wiesen auf einen intravenösen Drogenabusus mit Heroin hin. Im Röntgen-Thorax-Bild zeigten sich multiple Abszesse in beiden Lungenhälften. Auskultatorisch fand sich bei abgeschwächtem 1. Herzton ein 3/6 Sofortsystolikum über dem 4. ICR rechts parasternal, das ¡nspiratorisch zunahm. Das Echokardiogramm zeigte flottierende Vegetationen an der Trikuspidalklappe, von denen aus septische Embolien zur Abszeßbildung in beiden Lungen bei einer durch Staphylokokken induzierten Endokarditis geführt hatten.
