Zusammenfassung
Bei einem 37 jährigen Patienten stellen sich nach exzessivem Alkoholkonsum plötzlich starke Schmerzen in der Mitte des Oberbauchs mit Ausstrahlung in beide Flanken ein. Der Bauch ist prallelastisch gespannt („Gummibauch“). Der Patient ist kreislaufinstabil (Puls: 110/min, RR: 100/60 mmHg). Labor: Leukozytose (17.000 /µl), Anstieg der Serumamylase auf das Fünffache der Norm. Im Ultraschall ist der Kopf der Bauchspeicheldrüse vergrößert. Eine akute Pankreatitis wird diagnostiziert. Unter Intensivüberwachung erfolgt die Therapie mit i.v. Volumenzufuhr (6 I/24 h) und Analgetika. Unter der Schocksituation entwickelt sich ein akutes Nierenversagen.4 Wochen später haben sich alle Befunde normalisiert. Nach jahrelangem fortgesetztem Alkoholabusus stellt sich der Patient wegen chronischer Oberbauchschmerzen, Gewichtsabnahme und Diarrhoen erneut vor. Es finden sich eine Steatorrhoe und ein Diabetes mellitus. Sonographisch zeigen sich Verkalkungen in der Region des Pankreas. Diagnose: Chronische Pankreatitis mit endokriner und exokriner Insuffizienz. Die Diarrhoen gehen unter oraler Pankreasfermentsubstitution zurück. Der Diabetes mellitus wird mit Insulin behandelt.
