Zusammenfassung
Ein 52 jähriger Patient, bei dem schon länger eine fortgeschrittene Niereninsuffizienz auf dem Boden einer chronischen Glomerulonephritis bekannt war, wurde aufgrund akuter Verschlechterung seines Allgemeinzustandes mit ausgeprägter Dyspnoe in die Notaufnahme eines Krankenhauses eingewiesen. Bislang war sein Zustand unter hoch dosierter Gabe von Schleifendiuretika, sowie einer kalium- und phosphatarmen Ernährung kompensiert gewesen. Am Aufnahmetag hatte er jedoch eine größere Menge Kirschen gegessen. Im Aufnahmebefund fielen neben ausgeprägten Ödemen, eine pulmonale Stauung sowie eine Bradykardie auf. Im Elektrokardiogramm zeigten sich bei fehlenden P-Wellen breite, deformierte Kammerkomplexe. Laborchemisch bestand neben deutlich erhöhten Retentionsparametern (Kreatinin 9,9 mg%,Harnstoff-N 174 mg%) ein Serumkalium von 7,8 mmol/l sowie eine metabolische Azidose mit einem BE von -17,1 mmol/l. Unter einer umgehend eingeleiteten Notfalldialyse ließ sich das Serumkalium rasch normalisieren, im EKG zeigte sich wieder ein unauffälliges Bild. Nach Fortführung der Dialysebehandlung in den folgenden Tagen besserte sich der Allgemeinzustand des Patienten rasch.
