Zusammenfassung

Fall 1. Vorgestellt wurde eine 37 jährige Patientin mit bioptisch gesicherter fokal-segmentaler Glomerulosklerose und Vorbehandlung des nephrotischen Syndroms,zunächst erfolgreich mit Steroiden, drei Jahre später mit Steroiden in Kombination mit Cyclophosphamid. Unter dieser Therapie und einer effektiven antihypertensiven Behandlung gelang zunächst eine erneute Remission des nephrotischen Syndroms und ein nur langsamer, aber kontinuierlicher Anstieg der Retentionswerte. Aktuell lag das Serum-Kreatinin bei 5,5 mg/dl, BUN 79 mg/dl, Plasma-Bikarbonat 16,8 mmol/l, Kalium 5,2 mmol/l, Kalzium 2,0 mmol/l, Phosphat 2,1 mmol/l, Hämatokrit 29%, Proteinurie 6,5 g/24 Stunden.
