Zusammenfassung
Die Aussage, minimal-invasive orthopädische Eingriffe an der Schulter seien nicht mehr als Diagnostik durch das Schlüsselloch, ist überholt. Die großen Fortschritte in der bildgebenden Diagnostik (Arthro-CT, MRT, Sonographie etc.) haben die Diagnostik und Therapie an der Schulter grundlegend verändert. Sie haben die Grundlage zum Verständnis der Pathologie und Biomechanik des Schultergelenkes gelegt, wovon die moderne Schulterchirurgie profitierte. Nur in speziellen Fällen ist eine diagnostische Arthroskopie heute noch von Nöten. Beispielsweise um Zusatzinformationen beim operativen Vorgehen bei der Rotatorenman-schettenruptur zu erhalten, ist es von Vorteil, eine diagnostische Arthroskopie initial durchzuführen. Mit Hilfe dieser Beurteilung kann dann der am besten geeignete minimal-invasive Eingriff gewählt werden [2, 1, 3, 6, 20].
