Zusammenfassung
Die akute intermittierende Porphyrie (AIP) ist die häufigste akute Porphyrie. Symptomatische Patienten und asymptomatische Genträger weisen eine Reduktion der Aktivität des Enzyms Porphobilinogendesaminase (PBG-D) auf 50% auf, die für die Porphyrin- und auch die Hämoglobinsynthese ausreicht. Akute Porphyrieattacken treten auf, wenn die Hämsynthese durch Medikamente, Alkohol oder Infektionen gesteigert wird, die PBG-D die Vorstufen aufgrund ihrer Reduktion nicht adäquat umsetzen kann, so daß Porphobilinogen (PBG) und δ-ALA (δ-Aminolävulinat) akkumulieren, die in hohen Konzentrationen als neurotoxisch gelten. Die PBG-D kommt in 2 Isoenzymen vor (1 Gen →2 Enzyme), von denen eines in allen Geweben und das andere nur in Erythroblasten nachweisbar ist. Porphyrieanfälle präsentieren sich klinisch als neuroviszerale Beschwerden (z.B. akutes Abdomen) oder neurologische Ausfälle, die einen tödlichen Verlauf nehmen können. Bei Patien- ten mit AIP ist deshalb eine freiwillige Familienanalyse zur Identifikation präsymptomatischer Genträger sinnvoll, die mit Notfallausweisen versorgt und ausführlich über ihre Krankheit sowie Faktoren, die Attacken auslösen können, informiert werden. Die Identifikation von Genträgern ist heute mit der molekularen Gendiagnostik, die auf dem Prinzip der Kombination einer PCR-Amplifikation mit der denaturierenden Gradienten- gelelektropherese (DGGE) beruht, leicht durchführbar. Bei diesem Ansatz werden die einzelnen Exons und ihre Grenzen zu Introns amplifiziert und über ihre Wanderung in einem HarnstoffFormamid-Gradienten-Gel auf Mutationen untersucht. Mit dieser Methode sind bisher etwa 135 Mutationen bei der klassischen (Mutation im ubiquitären und im Erythroblastenenzym erkennbar) und 5 Mutationen bei der varianten AIP (Mutation nur im ubiquitären, aber nicht im Erythroblastenenzym erkennbar) identifiziert worden. Bisher konnten keine direkten Genotyp-Phänotyp-Beziehungen zwischen dem Typ der Mutation und dem Auftreten bzw. dem Schweregrad klinischer Sym- ptome gefunden werden. Dies spricht dafür, daß für die Penetranz zusätzliche, in Zukunft zu identifizierende Gene eine Rolle spielen müssen. Neue gentherapeutische Ansätze werden in Zukunft erlauben, gezielt Punktmutationen im PBG-D-Gen zu korrigieren.
