Zusammenfassung

Die PTCA stellt ein attraktives Konzept zur Behandlung der instabilen Angina pectoris und des akuten Herzinfarkts dar. Mit der PTCA kann nicht nur das Infarktgefäß wiedereröffnet, sondern auch die zugrundeliegende Stenose erweitert werden. Außerdem erhält man durch die Angiographie wichtige Informationen über den gesamten Koronarstatus, die die weitere Behandlungsstrategie (konservativ, interventionell oder operativ) bestimmen. Die Probleme der PTCA bei akuten koronaren Syndromen bestehen vor allem in Reokklusionen, vaskulären Komplikationen an den arteriellen Zugangswegen, der Zeitverzögerung bis zum Therapiebeginn und der Vorhaltung einer 24-h-Herzkatheterlaborbereitschaft.
