Zusammenfassung
Calciumantagonisten hemmen am Herzen und an der glatten Muskulatur den Einstrom von Calcium aus dem Extrazellulärraum während des Aktionspotentials. Dies führt zu einer Vasodilatation (vorwiegend der arteriellen Gefäße) und am Herzen zu einer Abnahme von Kontraktionskraft und Herzfrequenz, die allerdings durch eine adrenerge Gegenregulation infolge Vasodilatation kompensiert wird. Bei Calciumantagonisten vom Nifedipin-Typ (Dihydropyridine) bewirkt dieser Kompensationsmechanismus nicht selten sogar eine reflektorische Tachykardie. Weiterhin hemmen Calciumantagonisten vom Verapamil- und Diltiazem-Typ die AV-Überleitung und unter Umständen auch ventrikuläre Extrasystolen und Tachyarrhythmien. Die Abnahme von Herzkraft und Herzfrequenz einerseits und die Gefäßerweiterung andererseits sind qualitativ bei allen Calciumantagonisten gleich. Allen Calciumantagonisten gemeinsam ist auch, daß die Vasodilatation im Vergleich zur Kardiodepression bei niedrigeren Konzentrationen auftritt. Allerdings ist der Abstand zwischen vasodilatierend und kardiodepressiv wirkenden Konzentrationen bei Dihydropyridinen (z.B. Nifedipin) größer als bei Calciumantagonisten vom Verapamil- und Diltiazem-Typ (Verapamil, Diltiazem, Gallopamil).
