Zusammenfassung
Pilzinfektionen werden klinisch-diagnostisch und therapeutisch nach ihrer Lokalisation und der Art der Erreger unterschieden. Am häufigsten sind oberflächliche Mykosen der Haut und Hautanhangsorgane sowie der Schleimhäute. Organmykosen sind in unseren Breiten deutlich seltener, gewinnen aber bei Patienten mit erworbener Immunschwäche (AIDS) zunehmend an Bedeutung und sind auch im Rahmen einer immunsuppressiven Therapie zu beachten. Für Risikopatienten kann ferner auch die kommensale intestinale Mykoflora eine potentielle Gefahrenquelle sein. Ohne therapeutische Konsequenz ist sie jedoch—wie auch die übrige standorttypische kommensale Mikroflora—bei immunkompetenten Patienten. So läßt sich weder ein Zusammenhang zwischen einer Candidabesiedlung im Darm und Störungen wie Blähungen, Verdauungsbeschwerden, Roemheld-Syndrom, Herzbeschwerden, körperliche Schwäche, Ermüdbarkeit, Kopfschmerzen, Gelenkschmerzen, depressive Verstimmung etc. (in den 80er Jahren auch als „Candidiasis hypersensitivity Syndrome“ beschrieben, und heute als „Mykophobie“ bezeichnet) wissenschaftlich belegen, noch ist eine Eradikation der Hefepilze notwendig und möglich (Müller 1993, Rösch 1996, Scheurlen 1996, Seebacher 1996, Knoke 1998, Bernhardt 1998).
