Zusammenfassung

Die Lungentransplantation hat in den letzten 15 Jahren Eingang in die Therapie der terminalen respiratorischen Insuffizienz bei cystischer Fibrose gefunden. Die ersten erfolgreichen Herz-Lungentransplantationen wurden nach der Einführung von Ciclosporin A in den frühen Achtzigerjahren in Stanford bei Patienten mit primärer pulmonaler Hypertonie durchgeführt. Später entwickelte man in Toronto die Technik der Einzellungentransplantation für Patienten mit Lungenfibrose, woraus das Verfahren der bilateralen sequentiellen Doppellungentransplantation entstand. Seit den frühen Neunzigerjahren werden auch Lungenlappen von verstorbenen oder von lebenden Spendern transplan-tiert. Die kumulative Gesamtzahl der transplantier-ten Doppellungen, Einzellungen, Herz-Lungen und Lungenlappen liegt Anfang des Jahres 2000 weltweit bei 14000. In den USA weisen eine Reihe von Zentren eine Transplantationsfrequenz von mehr als 10 Lungen pro Jahr auf. Im deutschsprachigen Raum sind wor allem Hannover, Wien, Berlin , München, Zürich, Homburg and Kiel aktiv. Jährlich werden gegenwärttig in Deutschland allein zwischen 100 und 150 Lungen transplantiert
