Zusammenfassung
Jedes Jahr erkranken weltweit etwa 575000 Patienten an einem bösartigen Tumor im Kopf-Hals-Bereich [26]. Kehlkopfkarzinome machen etwa 1–2% aller bösartigen Tumoren aus und sind bislang die häufigsten Malignome im Kopf-Hals-Bereich [10]. In Deutschland wurden 1993 insgesamt 1672 Sterbefälle registriert [7]. Im Jahr 1993 werden für das Mundhöhlenkarzinom in Deutschland 1335 Sterbefälle angegeben [7]. Die Inzidenz dieser Tumoren steigt in vielen Ländern [13]. Männer sind häufiger betroffen, jedoch erkranken auch Frauen zunehmend wegen des steigenden Tabak- und Alkoholkonsums. In über 90% der Fälle handelt es sich beim Larynx- wie auch bei Mundhöhlen-/Oropharynxkarzinomen histologisch um Plattenepithelkarzinome. Das Durchschnittsalter der Patienten liegt bei 60,4 bzw. 55,9 Jahre; die 5-Jahresüberlebensrate beträgt 61% bzw. 38% (Mundhöhlenkarzinom: 41%). Bei 58% und 67% der Patienten mit einem Larynx- bzw. Mundhöhlen-/Oropharynxkarzinom kommt es zu einem Lokalrezidiv. Eine Lymphknotenmetastasierung wird in 18% bzw. 24% der Fälle registriert [7, 26, 27]. Diese Entwicklung zeigt, dass sowohl präventive Maßnahmen als auch der moderne medizinische Fortschritt bisher keine Verbesserung der Situation herbeiführen konnten.
