Einsatzmöglichkeiten der transösophagealen Echokardiographie in der perioperativen Überwachung

Zusammenfassung

Die kontinuierliche Überwachung des Herzens und der zentralen, herznahen Gefäße mittels transösophagealer Echokardiographie (TEE) ist ein etabliertes Verfahren der erweiterten perioperativen Überwachung des kardialen Risikopatienten [7,14,18]. Die TEE eröglicht intraoperativ ein kontinuierliches Monitoring der Ventrikelfunktion, eine hochsensitive Überwachung des Patienten auf Myokardischämien und eine optimierte Beurteilung der links- und rechtsventrikulären Vorlast (Füllungszustand, Volämie). Sowohl intra-als auch postoperativ ist die TEE ein wertvolles Hilfsmittel in der Differentialdiagnostik des hämodynamisch instabilen Patienten [4,22]. Die TEE ist ein semiinvasives, in Notsituationen rasch einsetzbares Verfahren. Die TEE liefert Informationen (anatomische Information, Information über Blutflüsse), die mit keinem alternativen Überwachungsverfahren zu erhalten sind. Untersuchungen haben gezeigt, dass die TEE im Operationssaal oder auf der Intensivstation bei 30–50% der Patienten neue, die Therapie wesentlich beeinflussende diagnostische Informationen liefert [4,11,18]. Im Folgenden sollen die Grundlagen der TEE und die wichtigsten Einsatzmöglichkeiten für den Anästhesisten vorgestellt werden.
