Zusammenfassung

Engpasssyndrome entstehen durch eine „relative Enge“, wenn ein Nerv durch eine vorgebildete physiologische Enge hindurchtreten muss und die abnormen Zug- und Druckbelastungen den Nerv an dieser Stelle besonders vulnerabel machen. Zusätzliche pathophysiologische Faktoren, wie entzündliche oder degenerative Prozesse, endokrinologisch bedingte Gewebezunahme (Hypothyreose, Akromegalie bei dem Karpaltunnelsyndrom!) spielen ebenso eine Rolle wie Narbenstrukturen, Traumafolgen (sog.„Ulnarisspätlähmungen“) oder Begleiterkrankungen wie eine Polyneuropathie. Engpasssyndrome treten in der Regel in der 2. Lebenshälfte auf. Die Diagnostik hat neben der Erhebung des klinischen Befundes einschließlich der Erfassung vegetativer Störungen (Ninhy-drin-Test) die Elektromyographie und Elektroneurographie zu umfassen, um seltene Innervationsanomalien nicht zu übersehen.
