Zusammenfassung
Maligne Tumoren im Kopf-Hals-Bereich führten im Jahr 1995 in der BRD zu etwa 6500 Todesfällen und machten bei Männern ca. 4,9% (fünfthäufigste Todesursache infolge Malignom) und bei Frauen ca. 1,2% aller tumorbedingten Todesfälle aus. Wie bei jüngeren Patienten steht auch im Alter in den frühen Tumorstadien die Operation im Vordergrund, wenn diese technisch und vom Allgemeinzustand des Patienten her möglich ist. In den lokal fortgeschrittenen Stadien sind die Ergebnisse der alleinigen Operation immer noch enttäuschend, weswegen verschiedene multimodale Therapieansätze untersucht wurden. Die postoperative Radiotherapie ist Bestandteil der Standardtherapie bei vielen Patienten entweder mit großem Primärtumor, ausgedehntem Lymphknotenbefall oder mit Risikofaktoren wie extrakapsulärer Lymphknotenmetastasierung oder inkompletter Resektion (Deutsche Krebsgesellschaft 2000). Mit einer primären präoperativen, d. h. neoadjuvanten Chemotherapie, werden bei lokal fortgeschrittenen Kopf-Hals-Tumoren sehr hohe Remissionsraten von 80% erzielt, die bisher aber nicht zu einer Erhöhung der Fünfjahresüberlebensraten geführt haben. Daher hat der neoadjuvante Therapieansatz (Radiotherapie, Radio-Chemo-Therapie, alleinige Chemotherapie) außerhalb von Studien derzeit keine Berechtigung. Abgesehen von der simultanen Radio-Chemo-Therapie ist eine systemische Chemotherapie fortgeschrittener Kopf-Hals-Tumoren heute nur dann indiziert, wenn die Erkrankung nach Operation und/oder Strahlentherapie lokal rezidiviert ist und/oder wenn Fernmetastasen aufgetreten sind. Diese Therapierichtlinien sind prinzipiell auf ältere Patienten zu übertragen. Jedoch müssen bestehende altersbedingte Komorbiditäten berücksichtigt werden, da sie zu Einschränkungen und Änderungen bei der Therapie führen können.
