Zusammenfassung
Das Bronchialkarzinom ist der häufigste zum Tode führende Tumor des Mannes. Weltweit muss mit 1 Mio. Todesfällen/Jahr gerechnet werden. In der Bundesrepublik Deutschland verstarben im Jahre 1995 31.147 Personen am Bronchialkarzinom. Die altersstandardisierte Sterberate lag bei Männern in den alten bzw. in den neuen Bundesländern in diesem Jahr bei 45,5 bzw. 53,7 Todesfällen/100.000 Personen. Für die Frauen betrugen die entsprechenden Raten 9,6 bzw. 7,6. Damit ist dieser Tumor die häufigste Krebstodesursache bei Männern mit einem Anteil von 26,5% (West) bzw. 27,6% (Ost) und die dritthäufigste bei Frauen nach Brustkrebs und Dickdarmkrebs (Becher u. Wahrendorf 1998). Kein anderer Tumor hat im Verlauf der letzten 5 Jahrzehnte so stark zugenommen wie das Bronchialkarzinom. Diese Zunahme betrifft weltweit beide Geschlechter. Noch ist das Bronchialkarzinom etwa 5-mal häufiger bei Männern anzutreffen als bei Frauen. Jedoch verschiebt sich das Geschlechterverhältnis kontinuierlich zu Ungunsten der Frauen. Diese Zunahme der vergangenen Jahrzehnte hat sich in der Bundesrepublik Deutschland in den vergangenen Jahren bei Männern nicht fortgesetzt. Hier ist eine gewisse Stagnation, sogar die Tendenz zur Abnahme erkennbar. Bei Frauen nimmt diese Krebsform jedoch kontinuierlich, zum Teil expotentiell zu. Die Inzidenzrate nimmt mit zunehmendem Lebensalter zu (Tabelle 29.1) und erreicht nach eigenen Analysen im Krankengut einer Spezialklinik (Thoraxklinik Heidelberg-Rohrbach, n = 5333) den Häufigkeitsgipfel in der Altersgruppe der 60 bis 70-Jährigen bei einem Median von 61 Jahren. 1064 (20%) der Patienten waren ≥70 Jahre. Hieraus ergeben sich bedeutende klinische Konsequenzen, da bei diesen Patienten mit einer Multimorbidität gerechnet werden muss, welche die therapeutischen Möglichkeiten erheblich einschränken können.
