Zusammenfassung
Die Intensität spezifischer Therapien wurde für den alten Tumorpatienten zumeist auf der Basis langjähriger onkologischer Erfahrungen jedoch ohne zuvorigen Nachweis einer Äquieffizienz gegenüber konventionellen Dosierungen reduziert. Diese Vorsicht wird durch die eingeschränkten Organfunktions- und Knochenmarkreserven begründet. Umgekehrt wäre aber zu prüfen, in wie weit eine Intensivierung supportiver Maßnahmen zur Prävention und Reduzierung alterspezifisch gesteigerter und schlecht zu beherrschender Toxizität beitragen kann. Studien zur Substitution von Blut- und Blutprodukten sowie zu antiinfektiösen Therapien würden aber nur dann klinische Relevanz bekommen, wenn reliable Tests zur Beurteilung des biologischen Alters und valide Messinstrumente zur Erfassung der altersspezifischen Lebensqualität verwendet würden. Diese stehen derzeit aber nicht zur Verfügung.
