Zusammenfassung
Die Bedeutung der Meniskuserhaltung ist in Anbetracht der schlechten Langzeitergebnisse nach Meniskektomie unumstritten. Bereits 1948 hat Fairbank die klassischen Folgen der offenen Meniskektomie beschrieben, d.h. die Abflachung der Femurkondylen, die Osteophytenbildung und die Verschmälerung des Gelenkspaltes [20]. Die Kontaktfläche zwischen Femur und Tibia nimmt nach Meniskektomien in Relation zum Ausmaß der Meniskusentfernung ab, wobei die Druckbelastung des Knorpels entsprechend zunimmt. Die Kontaktfläche zwischen Femur und Tibia halbiert sich durch den Meniskusverlust (normales Knie 1150 mm2, nach Meniskektomie 550 mm2, [6, 21]). Dies gilt natürlich auch für die partielle Meniskektomie. Hier sollte so sparsam wie möglich und so gründlich wie nötig reseziert werden. Gesundes Meniskusgewebe sollte erhalten und die Reparatur und Heilung von Meniskusverletzungen mit allen Mitteln unterstützt werden. Deshalb sollte die meniskuserhaltende Chirurgie von jedem arthroskopisch oder offen tätigen Kniechirurgen beherrscht und vor allem richtig indiziert werden können.
