Zusammenfassung
Intoxikationen unter Valproinsäure(valproic acid, VPA)-Therapie können sich trotz VPA-Blutspiegeln im sogenannten therapeutischen Bereich manifestieren. Zu unterscheiden sind leichte Intoxikationen (klinisch dominiert ein Haltetremor) von schweren Überdosierungen aus meist suizidaler Absicht oder nach versehentlicher VPA-Einnahme und akute oder chronische Enzephalopathien. Schwere Intoxikationen mit VPA-Blutspiegeln bis zum aofachen der Norm haben trotz einiger mitgeteilter Todesfälle insgesamt eine günstige Prognose. Klinisch dominiert ein Koma, im EEG eine Allgemeinveränderung. Enzephalopathien bei meist nur geringen VPA-Dosierungen gehen ebenfalls mit einer Beeinträchtigung der Bewusstseinslage, Tremor und Ataxie einher. Im EEG zeigen sich generalisierte hochamplitudige langsame Wellen. Die Ursache der Enzephalopathie ist unklar. Ein Anstieg des Phenobarbitalblutspiegels bei Kombinationstherapie wurde ebenso angeschuldigt wie eine Hyperammonämie oder eine direkte intrinsische Wirkung von VPA auf das Gehirn. Darüber hinaus könnte die Enzephalopathie auch Ausdruck einer Aktivierung epileptischer Aktivität durch VPA sein.
