Valproinsäure in der Behandlung des manischen Syndroms und in der Prophylaxe bipolarer affektiver Störungen

Zusammenfassung

In der Behandlung manischer Syndrome und in der Prophylaxe bipolarer affektiver Störungen wird seit den 5oer Jahren Lithium eingesetzt. Da Lithiumionen jedoch erhebliche Nebenwirkungen aufweisen (u. a. Tremor, Gewichtszunahme, Gedächtnisstörungen), ein Teil der Patienten (insbesondere mit „rapid cycling“ und dysphorischer Manie) schlecht auf eine Lithiumgabe respondieren, und in einigen Fällen ein diskontinuitätsbedingter Wirkverlust auftreten kann, wurden seit den 7oer Jahren Carbamazepin (CBZ) und seit den 8oer Jahren Valproinsäure (valproic acid, VPA) als Alternative zu Lithium eingesetzt. Durch klinische Studien gut belegt ist die Wirksamkeit von VPA beim akuten manischen Syndrom. In der Prophylaxe bipolarer Erkrankungen scheint VPA auch wirksam zu sein, obwohl hierzu kontrollierte Studien noch fehlen. Gegenüber CBZ hat VPA den Vorteil geringerer Nebenwirkungen und dem Fehlen einer Enzyminduktion. Insgesamt stellt VPA eine wertvolle Alternative in der Therapie bipolarer affektiver Erkrankungen dar.
